ASH Highlights | Claudio Cerchione, MD, PhD | ASH 2023

Hi, I am Claudio Cerchione. I work in Istituto Romagnolo per Osservio di Tupori di Navadori, located in Mendo, not far from Bologna, not that far from Italy. And I'm really happy to introduce you to highlights of ASH 2023 that is currently going in San Diego. I think the particular aspects in multiple monomers that the frontline setting in transplant eligible patients is going to change thanks to Perseus, CMN 17 and ISCHIA clinical trial that has been presented there. Dara BRD and this Atuxium KRD has shown incredible results in terms of overall response rate, progress of his survival, but particularly in deepness of response. MRD negativity, sustained MRD negativity is going to become the new endpoint and is the one that maybe will correlate in the next future with the cure that is the best wish that we give to our patients. In relapsed refractory setting, it was particularly exciting to see data from TALCATAMAB, Monumental 1 clinical trial in which not only the overall response rate of 70% but also the 50% of response in patients previously treated with other B-specific antibodies are particularly important and I think this will be a game changer in the therapeutic panorama of relapsed refractory setting. ALNUCTAMAB, a new B-specific antibody, CD3-BCMA-directed has shown also incredible results in terms of overall response rate, but also in terms of MRD negativity, 100% in evaluable patients, subcutaneous administration, wonderful tolerability and I think this will be one drug about which we will speak also thanks to going to start phase 3 trial against the considered the standard of care. Other important information, I stopped multiple meloma, the Iceland trial, showing how to monitor AMGAS and smoldering meloma has shown updates and I think this is a really interesting trial to prevent multiple meloma and I think in general the take home message is that in multiple meloma starting from frontline setting we don't have only to achieve a response but we have to achieve the deepest response as possible going to a new endpoint which is MRD negativity. Sustainable MRD negativity during the time will be the key point to reach the cure of multiple meloma and I'm sure that this will happen in the next future.
