How quickly can a patient see a response with CAR-T cell therapy?

How quickly can patients see a response with CAR T cell therapy? Similar to transplant, this is a one-time infusion. So you kind of expect that the treatment effect would be obvious right away. So some degree of response we see very early. In fact, in some patients, even if we would do a bone marrow biopsy 14 days after CAR T cells, we may see that all of the myeloma is gone. But myeloma is a complicated disease and we have a lot of these biochemical parameters that we measure in the blood. The M-spike, the light chain, kappa and lambda, we have various parameters that we follow. Those tend to go down gradually. And in fact, even three months is frequently not the perfect timing to see the best response. And we've had patients who had ongoing decrease in their M-spike or their light chain levels for many months, even up to one year after the infusion without any subsequent therapy. Some patients do go on maintenance after CAR T cells. This is not yet very well established on whether that's useful, worthy of doing. This has not really been studied. The majority of our patients are just monitored. Only if we have any concern that the disease is not responding optimally to the therapy, then we might put patients on some low-dose maintenance with agents that are the same agents that are used for maintenance after transplants, such as Revlimid or Pumilist. And some physicians use other myeloma agents for this purpose.
