Good morning, good morning, good morning. My name is Bruno Costa. I'm a palliative care fellow at the Mount Sinai Hospital and I'm a local research affiliate at Memorial Sloan Kettering and Mount Sinai City Cancer Institute. After large phase three trials such as CIFILS, PERSELS, and EMRAD, quadruplets have become the standard of therapy in patients with newly diagnosed multiple myeloma. Quadruplets that include immunomodulatory drugs, proteasome inhibitors, steroids, and now anti-CD38 monoclonal antibodies such as Iztuximab and Derotumumab. However, it's still not clear how the outcomes of CAR-T cell therapy may be impacted by derorefractory disease after exposure to those agents. In our study, we investigated whether patients with derorefractory disease versus deronorm refractory disease had different clinical outcomes after BCMA-directed CAR-T cell therapy for relapsed refractory multiple myeloma including Ida-cell, CILTA-cell, or Orova-cell. All of those patients were treated at Memorial Sloan Kettering and we did observe a non-significant difference in outcomes between groups, meaning that those groups had comparable progression fear survival, comparable overall survival, comparable response rates, and also similar rates of adverse events such as CRS and ICANN. However, longer follow-up is still needed to best clarify whether having derorefractory disease can negatively impact outcomes after CAR-T cell therapy. But the early evidence that we have is encouraging that this may not be the case.
