Can Sonrotoclax Improve Outcomes in t(11;14) Relapsed Myeloma? | Rakesh Popat, PhD

Hi, my name's Rakesh Popat and I'm a haematologist based at University College Hospital in London in the UK. At this year's ASH there's going to be a presentation about a new BCL2 inhibitor known as sonrotoclax in combination with a proteasome inhibitor, carfilzomib and dexamethasone. This is targeted for a patient group that have a T1114 translocation, so that's a specific genetic marker that we find from the bone marrow biopsy tests. What we know is that this specific genetic marker is found in about somewhere between 10 to 20% of patients, so there's a good chance that someone might have this. And what we know from previous studies is that a class of drug known as a BCL2 inhibitor can be very effective and I was really closely involved with a drug called venetoclax which was used for patients with myeloma. Now unfortunately venetoclax didn't get licensed on the background of the clinical trials that we ran, but what it did show was that this class of drug is effective. So this trial is using sonrotoclax which is a new and more advanced BCL2 inhibitor, has a slightly better side effect profile and is more potent. And it combines it with carfilzomib and proteasome inhibitors to try and enhance its effects. What we're showing is some very early data which shows that the combination of these three drugs seems to be safe. The side effect profile doesn't seem to raise any other concerns compared to what we know with carfilzomib and these types of drugs. And also interestingly we're finding that our patients are responding really well. And that's really good news because we don't have any targeted drugs available for myeloma specifically and if we can get these combination to work well then this may represent a new treatment pathway for these specific group. So still early data but I would say it's very encouraging that this combination may be effective.
