Transferring Your Care to Your Clinic
If you’re considering transferring to a different health program, there are a few steps to take. First, speak with your current clinic to start the process. They will confirm if you're eligible and provide a form that allows them to send your medical records to the new location. The transfer cannot begin until you sign and return this form. Transfers are typically allowed once per year and may take up to 30 days to finalize, with your first appointment at the new clinic possibly taking an additional 30 to 60 days.
To ensure a smooth transition, plan ahead. Schedule a final visit with your current clinic, especially if it's been over a year since your last exam or if you need treatment soon. Check for any pending documentation and refill any medications to avoid gaps in care. You should also follow up on any pending referrals or authorizations before your transfer is complete. During the transition, continue contacting your current clinic with any questions. Your new clinic will only be available once your transfer is officially finalized and confirmed in writing by the Health Program.
