Ibrutinib + Venetoclax (I+V) for CLL: Findings from CAPTIVATE & GLOW | Paolo Ghia, MD, PhD

My name is Paolo Ghia. I am a professor of medical oncology at Università Vita Salute San Raffaele Milano, Italy. At ASH this year in Orlando, we presented an abstract where we studied the genetic landscape of patients enrolled in the Captivate and Glow study. Just to recall you, the Captivate study is a phase 2 study where young patients and fit patients below 70 years of age were treated with ibrutinib plus venetoclax. Ibrutinib given for 3 months, associated then to venetoclax for 12 months. And the Glow study was instead a study for elderly unfit patients above 65 years of age, where IV, the combination of IV was compared to chlorambucid plus subbinotudum. And we already showed long-term follow-up, even more than 5 years of follow-up with sustained progression-free survival and very good responses and MRD responses, undetected MRD. In this study we wanted to go a little bit deeper in the analysis and therefore we wanted to understand if different genetic subgroups of patients were responding in a different way to the treatment, to the IV combination. And therefore we studied more than 1,000 genes in the Captivate population and we did whole exome phenotyping in patients enrolled in the Glow, in a total of more than 500 patients. And at the end of the day, just to make a long story short, we showed that there were no genetic subgroups that were negatively affected when treated with i plus V, with the exception that we already showed in the past, that patients with unmutated immunoglobin genes tend to have a shorter patient-free survival, though it still not reached 5.5 years. And the patients with P53 abnormalities who were enrolled only in the Captivate study, those are the ones that have a shorter PFS unless they reach undetected MRD.
