If we are able to identify and target someone's AML genetics, why is AML still difficult to get rid of? All cancers are really expressing an advanced accelerated evolution. And so the only desire of a cancer cell is to grow and divide and survive. When you throw chemotherapy or targeted agents at a cancer, while it's probably killing the vast majority of cells, you're also selecting out for a cell that can resist and survive. And that survival likely is going to require some other genetic event to allow that to occur. That might be a cytogenetic event. That might be a DNA mutation event. That might be an epigenetic event where different genes are silenced or different genes are turned on that shouldn't be turned on. But these events are allowing that small population of cells to survive the attack and the onslaught of the chemotherapy or other therapeutic intervention. And so the other analogy people tend to use is whackable. You try to get rid of all the leukemia, all the cancer that you can see, but something else pops up in the setting of that treatment. And that really is accelerated evolution of this cell that's just trying to find a way to survive. We have to outsmart that cell. And so one way that we're starting to think about it is not only what are the mutations that a leukemia may have, but given that profile, what are the likely mutations that it may pick up to try to promote resistance? And can we preventatively try to treat to prevent that resistance mechanism from being effective?
