What is the role of a physical therapist in developing an individualized exercise program?

Physical therapists are trained in developing a very individualized program for patients. They have knowledge about anatomy, about pathophysiology, physiology. They can explain to a patient which exercises are good for them, which exercises might actually be harmful. So working with a physical therapist is very helpful, and they have the knowledge what to do and how to improve, because doing always the same might not be the right way. They might explain also what can be an improvement, what can be more challenging exercises that then leads to benefits for the patients.

Here at MGH, we have our own lifestyle medicine program. It's a multidisciplinary program where we have a medical oncology provider, whether an M.D. or nurse practitioner, a nutritionist. We have dedicated physical therapists who work with us and behavioral psychologists to do wellness programs. You don't need all that. So your doctor can work with you if this is something they have expertise in and feel comfortable doing. But physical therapists are really very important.

One of our other providers here is a doctor in physical medicine, rehabilitation. So, PM&R, we have someone who specializes in cancer here. Those doctors are also available. But I think talking to your oncologist, seeing what resources are available and using that as your starting point is a good way to go if you have myeloma.

Are there special certifications for people who work with cancer patients on developing a physical fitness routine?

There are additional certifications for people who really want to be involved with cancer patients. It's wonderful when someone has a vested interest in that means they also may have expertise in taking care of cancer patients. There are a variety of different problems that arise in different cancer populations that physical therapists like. For women who have breast cancer, for example, that can have, you know, decreased range of motion. Those types of providers may be more comfortable dealing with the limitations of cancer patients and could be a great asset to a patient, but they're not necessary.
