[Music] what is the plasmacytoma plasmacytoma what is the plasmacytoma that's an aggregation of plasma cells in one area so it's like a big ball of plasma cells and it can be either within the bone but often outside the bonds so you'll see them in an aggregation that's what we call extramedullary so an extra merrily plasmacytoma is an aggregation of plasma cells not in the bone and you can still see a plasmacytoma that's in the bone that say for example grows out it's sometimes not that bad if you only have one of these things and nothing else if you were to develop a single plasmacytoma inside the bone it's called a solitary plasmacytoma and those often can be treated very conservatively and managed locally often with radiation and then close follow-up there are some features that would suggest perhaps these might progress perhaps on molecular testing or finding minut amounts of normal plasma cells in the marrow so long story short early on plasmacytoma can be very limited multiple plasma cells homas can be a bad prognostic sign especially in the context of recurrence where they're multiple plasmacytoma is that pop up outside of the bone there are often difficult control if they're in organs that normally don't have plasma cells such as the liver or the skin or other organs where you just don't usually see this plasmacytoma x' are basically the solid tumor version of multiple myeloma the plasma cells that are part of multiple myeloma can either spread throughout the bone marrow or form single tumors and so some patients will have multiple myeloma and we'll also see solid tumor versions or plasmacytoma x' that can show up pretty much anywhere they want they're most commonly seen in the bone or near the bone because we know that myeloma likes to damage the bone some people will have what we call a solitary plasmacytoma they don't actually have multiple myeloma but they have just a single tumor made out of plasma cells made out of myeloma cells and in those patients who have only that disease sometimes they can be treated just with radiation alone they don't require systemic chemotherapy and sometimes can be cured of their disease just with radiation [Music]
