What is included in a survivorship care plan? So the first section usually has to do with evaluating the kind of treatments that you've had, the chemotherapies, for instance, or surgeries or radiation that you have had. What we do then is we build out tailored educational information based on those drugs and the treatment surgeries or radiations that you have received and our research as to what are the short and long-term side effects of those treatments. And then it produces a personalized document based on those treatments that you've received and tells you about some of the things that you should be aware of. So for example, say you are a patient with Hodgkin's disease who's had radiation to your chest wall. Well, the content that would be produced there would have something to do with cardiac toxicity and having monitoring for cardiac side effects both short and long term. The second section really has to do with adjusting to life with and after cancer. That's all those psychosocial things that we talked about from financial toxicity to Relationships after cancer, fertility, starting a family after treatment. And then the final section has to do with that healthy living after cancer and the recommendations for your follow-up care that you need as well as the screenings and examinations that are recommended for you to have for general cancer prevention.
