When most people think of lung cancer, they picture an older person who smoked for many years. But doctors are seeing a new trend: more young adults who have never smoked are getting diagnosed with lung cancer.

Why is this happening, and how does cancer behave differently in younger bodies?

A major study led by researchers at the Sylvester Comprehensive Cancer Center at the University of Miami Miller School of Medicine, along with experts from Labcorp and Dana-Farber Cancer Institute, provides clear answers.

The landmark study looked at data from 14,246 people with non-small cell lung cancer (NSCLC). The findings reveal that lung cancer in young adults is biologically different from lung cancer in older adults.

Most importantly: younger adults are much more likely to have treatable genetic changes that doctors can target with specialized medicine.

The research team compared tumor samples across different age groups and found a clear split in how the cancer is built:

  • Younger Patients Have "Targetable" Mutations: Nearly 58% of younger adults had genetic changes in their tumors that match existing, FDA-approved targeted therapies. In comparison, only about 45% of patients aged 55 and older had these treatable changes.

  • The Most Common Genes in Young Adults: Younger patients were far more likely to have mutations in genes called EGFR, ALK, and ROS1. These genes act like hyperactive light switches, causing cells to grow out of control.

  • Older Adults Have Different Patterns: Older patients were more likely to have changes in the KRAS gene and a higher Tumor Mutational Burden (TMB). This means their tumors contained more accumulated DNA damage over time.

The authors emphasized that tumor biology doesn't change overnight on a specific birthday. Instead, there is a gradual shift in tumor genetics over a person's lifespan.

One of the biggest takeaways from this research is that every young adult diagnosed with lung cancer needs comprehensive biomarker testing.

Biomarker testing (also called genomic profiling) acts as a genetic roadmap of the tumor. Without this test, doctors might miss an actionable mutation, potentially delaying access to effective targeted treatments.
