Do your part to accelerate myeloma research by participating in one of our health history surveys. These surveys help researchers understand a better overall picture of their patients' current health, health history, and quality of life. 

Completing these surveys also helps HealthTree Foundation understand your current health, so that we know what personalized treatment recommendations and clinical trials to give to you in your HealthTree Cure Hub profile. This is a huge benefit that can simplify your treatment plans for multiple myeloma. 

By contributing to these surveys, you are adding important information to our research database. The more information that researchers have, the more accurate their data pulls will be when analyzing myeloma patients, their health, and their outcomes. 

Listed below are seven health surveys with details about each one. You can choose to take one or more of these surveys to contribute to our patient database.

How Can I Take These Surveys?

My Current Health

  • Description: This is a HealthTree Survey about your current health status. We ask that you please keep this updated as your health condition changes over time. 
  • What it asks: this survey asks about any heart, lung, bone, kidney, diabetes, blood pressure, blood clot (DVT), neuropathy, infections, and anxiety conditions.  It also asks about your ability to walk/exercise, shop for groceries, or prepare meals without help, whether or not you have a caregiver,  your stem cell transplant eligibility, and the current status of your myeloma. 
  • Time: This survey should take you about six (6) minutes. 

Health History: Heart and Lungs

  • Description: A HealthTree survey regarding your cardiovascular and pulmonary system health history. 
  • What it asks: this survey asks about your history of any cardiovascular disorders including hypertension, abnormal heart rhythms or arrhythmia, Deep venous thrombosis (DVT), brain stroke, pulmonary embolism, heart failure, myocardial infarction, coronary artery disease, peripheral artery disease, aneurysm or other blood clot conditions. It also asks about your history with pulmonary disorders such as asthma, chronic obstructive pulmonary disease (COPD), pulmonary emphysema, sleep apnea, and recurrent pneumonia. 
  • Time: This survey should take you about seven (7) minutes. 

Heart and Metabolic Family History

  • Description: A HealthTree survey about your family health history. 
  • What it asks: This survey asks about your family history with cardiovascular disease such as high blood pressure, heart attack, heart failure, arrhythmia, atrial fibrillation, heart valve complication, stroke, pulmonary embolism, deep vein thrombosis (DVT), peripheral vascular disease, or other health complication. It also asks about any family history of metabolic disease such as diabetes type 1 or type 2, metabolic syndrome, obesity, hypercholesterolemia, or another metabolic disease. 
  • Time: Depending on your answers, this survey could take you up to 8 (eight) minutes. 

Cancer Family History

  • Description: A HealthTree survey about your family's history with cancer. 
  • What it asks: This survey asks about your family's history with any of the following cancers: bladder, breast, brain, colon, colorectal, Hodgkins disease, kidney, leukemia, liver, lung, lymphoma, melanoma, mouth/throat, myeloma, smoldering myeloma, MGUS, pancreatic, prostate, rectal, sarcoma, stomach, thyroid,   uterine/endometrial, unknown, other, cervical, or ovarian.  
  • Time: Depending on your answers and knowledge of cancer family history, this survey could take you up to 10  minutes. 

Health History: General and Immune

  • Description: A HealthTree survey about your blood type, allergies, autoimmune conditions, or chronic infections.
  • What it asks: This survey asks you to identify your blood type, if you have been diagnosed with connective tissue disease, ulcer disease, any food, drug, insect, latex, mold, pet, or pollen allergies, any autoimmune disorders such as AIDS, autoimmune anemia, cerebrovascular disease, Hashimoto's thyroiditis, Grave's disease, psoriasis, lupus, inflammatory bowel disease, Raynaud's syndrome, scleroderma, Sjögren's syndrome,  MS (multiple sclerosis), Fibromyalgia, or another autoimmune disorder. Finally, it asks about any chronic infections such as HIV, Hepatitis B or C, mono, COVID-19, shingles, or another chronic infection. 
  • Time: This survey will take you about 14 minutes to complete. 

Health History: Kidney, Intestines, Skeletal, etc.

  • Description: A HealthTree survey about related health conditions including diabetes, thyroid, kidney, gastrointestinal, neurological and musculoskeletal issues (including osteonecrosis of the jaw).
  • What it asks: This survey asks if you have ever been diagnosed with diabetes, hyperlipidemia, thyroid disease, kidney,  gastrointestinal, neurological, or musculoskeletal disorders. 
  • Time: Depending on your answers, this survey could take you up to 15 minutes. 

Health History: Surgeries, Other Cancers, Mental Health:

  • Description: A HealthTree survey about past surgeries, other cancer diagnoses you may have, and any other related mental health concerns.
  • What it asks: This survey asks if you have had any of the following surgeries: appendectomy, gallbladder removal, hysterectomy, hernia repair, mastectomy, C-section, prostatectomy, cataract removal, joint replacement, broken bone repair, heart bypass surgery, stent procedure, angioplasty, or any other surgeries. It also asks about other non-myeloma cancer-related diagnoses like basal cell carcinoma and the other cancers listed above. Finally, it asks about mental health disorders such as depression, anxiety/panic attacks or PTSD,  insomnia, or other mental health conditions.  
  • Time: Depending on your answers, this survey could take up to 30 minutes of your time.
