At this ASH meeting, I’m privileged to present a large collaborative experience of myeloproliferative neoplasm (MPN)–related splanchnic vein thrombosis. These are blood clots that occur in the liver and are overrepresented in patients with MPNs.
This was an international collaboration, with over 900 patients’ data contributed from more than 20 sites around the world. Splanchnic vein thrombosis is a rare but very debilitating complication of MPNs, and historically, there has been limited data to guide treatment.
In this study, we asked a specific question: does giving medications to lower blood counts prevent these clots from recurring or prevent other complications?
Our analysis shows that, after adjusting for multiple factors, the answer is yes. Cytoreductive therapies that lower blood counts are associated with a reduced risk of clot recurrence, particularly recurrence or extension of blood clots in the liver itself. This is the first time this has been clearly demonstrated—previous analyses had not shown this.
These findings suggest that if a patient develops a blood clot around the liver with an MPN, even with normal blood counts, cytoreductive therapies can be an effective strategy to prevent future clots. We hope this information is useful to clinicians and provides hope to patients who experience this rare and serious complication.
