Personalizing Treatment with On-body Delivery of Isatuximab | Claudio Cerchione, MD | #EHA2025

Hello, I'm Claudio Cercione, working in Istituto Romagnolo per lo Stoio dei Tumori di Namadori, located in Melbourne, northern part of Italy. At THA 2025 in Milan, the therapeutic revolution is continuing and the personalisation of therapies doesn't mean only a target therapy with a new target-driven approach, but also personalising the formulation of the drugs and in general the schedule and administration. I think that one of the most important data comes from Heraklia, the comparison between endomenos and subcutaneous is atuximab. The atuximab is performed in the experimental arm with an on-body delivery system. The on-body delivery system means a button that we can put on the abdomen of the patient in which the drug is injected with an absolute reduction of infusion reaction and in some way all the problems that can come with the administration of the drug. In this trial we have seen not only an auto-inferiority in terms of effectiveness, but a great superiority in terms of tolerability, in terms of quality of life and particularly also with the satisfaction of the nurses. There is also ongoing a part of the trial with the decentralised trial, the possibility to have home nursing, so to take is atuximab at home. I think this will be a very good door to the new administration and the new schedules of the drug with the new trials, particularly from frontline setting. We know that is atuximab is becoming a new backbone in transplant eligible thanks to GMMG trial, but also transplant ineligible, LIMROTS and many other opportunities which we are using today. In my country it is the standard of care in lenalidomide refractory and it is the 38th naive patient thanks to eCaMA trial, but we are also performing together with pomalidomide in patients that are naive for both the drugs. So I think that Heraklia is an outstanding novelty not only for the study itself, but also with the potential application driven from these results. It is atuximab becoming subcutaneous, changed totally in my opinion the panorama of next future of multiple meloma. And thanks to this novelty we are going also to impact positively on the quality of life and this means to maintain the compliance, in some way the effectiveness of the treatment maintaining those events and those intensity. All these novelties to take to the cure of multiple meloma, the best wish that I always give to our patients, to their caregiver and to all meloma researchers. Thanks for the attention.
