Pancreatic Cancer Survival Rates: Life Expectancy, Stages, and Statistics

After a pancreatic cancer diagnosis, it is natural to want to know what the numbers say about survival. Statistics can be helpful for setting realistic expectations and having honest conversations with your care team. But they are also genuinely difficult to sit with, because pancreatic cancer statistics are among the most sobering of any cancer. At the same time, they describe populations, not individuals. Some people do far better than the numbers suggest, particularly those diagnosed at an early stage or whose tumors have specific molecular features that respond to treatment. Understanding what the statistics mean, and what they do not mean, is an important part of putting them in proper perspective.

How Common Is Pancreatic Cancer?
Pancreatic cancer is the tenth most common cancer in the United States in terms of new diagnoses, but it is the third leading cause of cancer death, behind only lung and colorectal cancer. This gap between incidence and mortality reflects just how serious this cancer is.
According to the American Cancer Society, an estimated 66,440 Americans were expected to be diagnosed with pancreatic cancer in 2024, and approximately 51,750 were expected to die from it. This means that the number of deaths is almost as large as the number of new diagnoses in any given year, which is very different from cancers, where effective treatments allow most people to survive.
Pancreatic cancer is slightly more common in men than in women, and rates are higher in Black or African American individuals than in other racial and ethnic groups. The risk increases sharply with age, with most diagnoses occurring in people between 65 and 74.
Researchers have projected that pancreatic cancer could become the second leading cause of cancer death in the United States by 2030 if current trends continue, which is one reason it receives increasing attention and research funding.
What Is the Survival Rate for Pancreatic Cancer?
Survival rates for pancreatic cancer are reported as five-year relative survival rates, meaning the percentage of people who are still alive five years after diagnosis compared to people of the same age without the disease. These statistics come from the National Cancer Institute's SEER (Surveillance, Epidemiology, and End Results) database and reflect patients diagnosed several years ago.
The overall five-year survival rate for pancreatic cancer is approximately 13 percent, making it one of the cancers with the lowest survival rates. However, this overall number masks significant variation depending on the stage at which the cancer is found.

The striking difference between localized and distant disease is one of the strongest arguments for earlier detection. Unfortunately, only about 10 to 15 percent of patients are diagnosed with localized disease, because early pancreatic cancer so rarely causes symptoms. About 50 to 55 percent of patients are diagnosed when the cancer has already spread to distant organs.
Important note: These statistics are based on patients diagnosed several years ago and do not reflect the most recent advances in treatment. Newer chemotherapy combinations and immunotherapy approaches are being studied, and outcomes for some patient subgroups have improved. Statistics describe what happened to large groups of patients in the past. They do not predict what will happen to any individual.
Is Pancreatic Cancer Curable?
The answer depends on the stage and the type of pancreatic cancer. For pancreatic ductal adenocarcinoma (PDAC), which is the most common type, surgical removal of the tumor (called resection) offers the only realistic chance of cure. However, only about 15 to 20 percent of PDAC patients are eligible for surgery at the time of diagnosis, because most tumors have already grown into major blood vessels or spread to distant organs.
For those who do undergo surgery followed by chemotherapy, meaningful long-term survival and cure are possible, though recurrence remains common. Five-year survival rates for surgically resected pancreatic cancer patients are in the range of 20 to 30 percent, with some favorable subgroups doing better.
Pancreatic neuroendocrine tumors (PNETs), particularly when they are low-grade and detected early, are considerably more curable. Many PNETs diagnosed at an early stage can be surgically removed with curative intent, and patients with PNETs generally have much better survival rates than those with PDAC.
For locally advanced pancreatic cancer (tumors that have grown into blood vessels but have not spread to distant organs), the goal of treatment has historically been disease control rather than cure. However, advances in chemotherapy and radiation are making more of these tumors resectable after initial treatment, a process called conversion therapy, and some patients who were initially inoperable have achieved long-term disease control or cure through this approach.
For metastatic PDAC, treatment focuses on controlling the cancer and maintaining quality of life for as long as possible, rather than cure. However, researchers are actively studying new approaches, including immunotherapy, targeted therapies, and cancer vaccines, that may one day change this picture. Clinical trials offer the most direct access to these emerging treatments.
