THE QUESTION:
When should MDS patients start EPO treatment? Should doctors start it early to prevent anemia, or wait until hemoglobin drops dangerously low?
THE TRIAL:
- 84 patients enrolled
- Early arm: EPO started immediately
- Delayed arm: EPO started when hemoglobin fell below 9 g/dL (near transfusion threshold)
KEY FINDINGS:
- Early EPO did NOT prevent transfusion dependence faster
- However, early EPO had much higher response rates (85% vs 55%)
- No difference in overall survival
- Similar quality of life between groups
- Slightly less anxiety in early treatment group
THE BREAKTHROUGH:
A new biological score combining EPO level and IPSS-M mutations can identify which patients will benefit most from early EPO treatment with higher response rates and delayed need for transfusions.
THE BOTTOM LINE:
Early EPO is beneficial for selected low-risk MDS patients, particularly those identified by the new biological score.
