They will confirm if you're eligible and provide a form that allows them to send your medical records to the new location. The transfer cannot begin until you sign and return this form. Some insurance companies and clinics have limits on how many times you can transfer care within the year. 

To ensure a smooth transition, plan ahead. Schedule a final visit with your current clinic, especially if it's been over a year since your last exam or if you need treatment soon. Check for any pending documentation and refill any medications to avoid gaps in care. You should also follow up on any pending referrals or authorizations before your transfer is complete. During the transition, continue contacting your current clinic with any questions. Your new clinic will only be available to help you once your transfer is officially finalized and confirmed in writing by the Health Program and you have been seen by the new health care team.
