What is hairy cell leukemia?

00:00:12:08 - 00:00:18:00
Hairy cell leukemia is a very rare form of adult leukemia.

00:00:18:02 - 00:00:20:15
Less than 2% of leukemias

00:00:20:18 - 00:00:23:08
are reported as hairy cell leukemias.

00:00:23:11 - 00:00:25:21
It's chronic or slow growing

00:00:25:21 - 00:00:30:01
leukemia of the cells, blood cells called white cells.

00:00:30:04 - 00:00:31:07
White blood cells.

00:00:31:10 - 00:00:33:08
White blood cells are of different types.

00:00:33:11 - 00:00:36:18
The specific type of white blood cell that's affected

00:00:36:21 - 00:00:39:19
in hairy cell leukemia is a B lymphocyte.

00:00:39:22 - 00:00:44:05
So it's a chronic leukemia of the B lymphocytes.

00:00:44:07 - 00:00:47:04
Where does the name hairy cell leukemia come from?

00:00:47:07 - 00:00:49:15
this is actually very, very familiar

00:00:49:15 - 00:00:53:17
for medical students because it's a very important broad question for them.

00:00:53:19 - 00:00:58:16
So when you look at the peripheral blood, the blood smear of patients

00:00:58:19 - 00:01:04:09
with hairy cell under the microscope, these cells are these B lymphocytes.

00:01:04:13 - 00:01:07:17
They look like regular B lymphocytes with size and shape,

00:01:07:21 - 00:01:11:18
but they have like these hairy projections coming out of the surface.

00:01:11:23 - 00:01:14:06
So that's where the name comes from.

00:01:14:06 - 00:01:15:12
The hairy cells.

00:01:15:15 - 00:01:18:22
What is the difference between hairy cell leukemia and CLL?

00:01:19:00 - 00:01:23:10
So CLL and hairy cell leukemia are both chronic leukemias but

00:01:23:13 - 00:01:28:23
they are different. They affect the same cell, the B lymphocyte.

00:01:29:02 - 00:01:34:09
But they're different like causes are different presentations.

00:01:34:14 - 00:01:37:18
So when you look at the cells,

00:01:37:20 - 00:01:41:12
the kind of markers they express, that's how we differentiate between them

00:01:41:16 - 00:01:43:17
which B cell they're both affect

00:01:43:17 - 00:01:49:03
B cell, which B cell is a hairy cell, which B cell is a CLL cell.

00:01:49:06 - 00:01:52:00
And these markers are on their surface

00:01:52:00 - 00:01:54:21
and they're detected by what's called flow cytometry.

00:01:55:00 - 00:01:58:22
For example, CLL cell is CD5 positive.

00:01:58:23 - 00:02:03:09
Hairy cell leukemia is CD5 negative and CD10 negative.

00:02:03:14 - 00:02:05:21
And it expresses specific markers like

00:02:05:21 - 00:02:11:06
CD11 c, CD103, CD125, CD25.

00:02:11:11 - 00:02:15:10
These markers are not expressed on CLL cells.
