How it is administered

Dexamethasone is available in several forms for administration:

  • Oral tablets: Common strengths include 0.5 mg, 0.75 mg, 1 mg, 1.5 mg, 2 mg, 4 mg, and 6 mg.
  • Oral solution: Available as 0.5 mg per 5 mL or as a concentrated solution (1 mg/mL).
  • Injectable form: Dexamethasone sodium phosphate can be given by intramuscular or intravenous injection.
  • Ophthalmic (eye) drops: Used for eye conditions, not for systemic blood cancers.

For blood cancers like large B cell lymphoma, the oral or injectable forms are most commonly used.

How it works

Dexamethasone is a synthetic corticosteroid (steroid hormone) that mimics the effects of hormones produced naturally by your adrenal glands. It works by reducing inflammation and suppressing the immune system. In the context of blood cancers, such as large B cell lymphoma, dexamethasone is used primarily for its anti-inflammatory and immunosuppressive effects.

By dampening the immune response, dexamethasone can help reduce the side effects of chemotherapy, such as allergic reactions or swelling. It can also help decrease inflammation caused by cancer or its treatment. Additionally, dexamethasone may be used to help manage symptoms like nausea or to treat complications such as swelling around tumors (for example, in the brain).

Serious allergic reactions are rare but possible. Always report any unusual symptoms to your healthcare provider.

Who should take it

Dexamethasone is used in the treatment of various conditions, including blood cancers like large B cell lymphoma. In these cases, it is often used as part of a combination chemotherapy regimen or to manage symptoms and side effects of cancer or its treatment.

Patients with large B cell lymphoma may receive dexamethasone to help control inflammation, reduce allergic reactions to chemotherapy, or manage complications such as swelling in the brain or other tissues. It may also be used in the palliative care setting to improve quality of life by reducing symptoms.

Who should not take it

Dexamethasone should not be taken by individuals who:

  • Have systemic fungal infections.
  • Are allergic to dexamethasone or any of its ingredients.

Caution is needed in patients with active infections, as dexamethasone can suppress the immune system and increase the risk of infections. It should also be used carefully in people with certain conditions such as diabetes, high blood pressure, osteoporosis, peptic ulcers, or psychiatric disorders, as it can worsen these conditions. Vaccination with live or live-attenuated vaccines is not recommended during treatment with immunosuppressive doses of dexamethasone.

Commonly used with

In the treatment of large B cell lymphoma and other blood cancers, dexamethasone is often used in combination with other chemotherapy agents such as:

  • Cyclophosphamide
  • Doxorubicin
  • Vincristine
  • Rituximab

It may also be combined with anti-nausea medications, antibiotics, or other supportive care drugs to manage side effects and complications.

Commonly tested with

Dexamethasone is frequently tested in clinical trials in combination with various chemotherapy regimens for blood cancers, including large B cell lymphoma. It is also studied with targeted therapies and immunotherapies to assess its impact on treatment effectiveness and side effect management.

Common combinations include dexamethasone with rituximab-based regimens (such as R-CHOP) and with newer targeted agents in clinical trials.
