LBCL Financial Resources

Patients and their caregivers should meet with the financial counselor at the facility where they are being treated. Each hospital will have its own financial counselor within each specific clinic (like the lymphoma clinic). The counselor will help determine coverage and estimate out-of-pocket costs. There may be restrictions on treatment options covered by Medicare and Medicaid. After you have met with the counselor, the resources below can help bridge financial gaps.

  • Baxter Healthcare: 1-800-548-4448 option 2 for cyclophosphamide (Cytoxan).
  • Bristol Myers Squibb:
    • For the CAR T-cell therapy liso-cel (Breyanzi), use the Cell Therapy 360® program or call 1-888-805-4555. 
    • For BMS treatments that are not CAR-T, use BMS Access Support®. These may include those like lenalidomide (Revlimid). 
  • Genentech: 1-888-941-3331 for rituximab (Rituxan) and polatuzumab vedotin-piiq (Polivy). 
  • Kite Pharma: 1-844-454-5483 for the CAR-T axi-cel (Yescarta). 
  • IncyteCARES. 1-855-452-5234 for tafasitamab-cxix (Monjuvi). 
  • Nextgen Pharmaceuticals: 866-585-2017. Financial assistance through AmeriPharma for oxaliplatin.
