Venetoclax + Ibrutinib Overcomes Drug Resistance in CLL: Long-Term Results #CLL

Dr. Kerry Rogers from Ohio State University presents long-term results from a phase 2 study showing that combining venetoclax with ibrutinib can help CLL patients whose disease develops resistance to BTK inhibitors.

THE CHALLENGE:
Some CLL patients on BTK inhibitors (like ibrutinib) eventually develop mutations that help cancer outsmart the drug. When this happens, patients typically stay on treatment indefinitely.

THE APPROACH:
Instead of switching to a different drug, researchers tried ADDING venetoclax (a BCL2 inhibitor) to the ibrutinib to overcome the resistance.

KEY RESULTS:
- Median progression-free survival: 40 months (vs 24 months with venetoclax alone)
- About 50% of patients achieved remission deep enough to STOP treatment
- Patients who stopped treatment stayed off medication for ~2 years on average
- Patients got time off medication to enjoy their lives

WHY IT MATTERS:
This strategy gives CLL patients hope for treatment breaks instead of lifelong medication. The combination approach works better than switching drugs alone.

FUTURE:
New studies now testing this strategy with newer BTK inhibitors (like pirtobrutinib) combined with venetoclax for finite treatment periods.
