However, ongoing global manufacturing changes and supply chain disruptions have led to shortages of key interferon therapies, requiring patients, families, and healthcare teams to adapt care plans.
What are interferons?
Interferons are naturally occurring proteins produced by the immune system to defend against infections and regulate cell growth. In cancer care, synthetic versions are administered via injections to help control abnormal blood cell production.
Specifically, interferons:
Inhibit cell multiplication: They "interfere" directly with the division and spread of abnormal cancer cells.
Stimulate immune defense: They train the body's immune cells to recognize and destroy mutant stem cells in the bone marrow.
Provide long-term control: Formulations like pegylated interferons are designed for long-lasting disease management.
Current Pegasys Shortage and Supply Updates
A major challenge facing MPN care is the ongoing availability of Pegasys (peginterferon alfa-2a).
Shortage timeline: The sponsor, Echo Therapeutics, notified health authorities of shortages in pre-filled syringes (135 mcg/0.5 mL and 180 mcg/0.5 mL strengths) due to global manufacturing site transitions. Supply disruptions are expected to continue through September 2026.
Access programs: To maintain access, regulatory bodies have approved the supply of overseas-registered Pegasys pre-filled syringes under Section 19A. While Australian-registered 135 mcg products traditionally come in a 4-pack, overseas substitutes are supplied as single pre-filled syringes.
Home Delivery Models: In regions like Australia, Pegasys is currently distributed exclusively through a dedicated Home Delivery Program to help manage supply limits.
Strength Changes: Availability between 135 mcg and 180 mcg strengths may fluctuate. If your doctor recommends switching doses based on what is in stock, a new prescription is required.
Alternative interferon options
With standard pegylated options facing supply constraints, healthcare providers are looking to alternative therapies:
Besremi (ropeginterferon alfa-2b-njft): Approved by the FDA for polycythemia vera (PV), Besremi is a long-acting pegylated interferon designed for administration under the skin once every two to four weeks. While it is not formally approved for ET or PMF, specialists may consider off-label use in specific clinical situations.
Non-pegylated interferons: Traditional non-pegylated interferon alfa-2a was discontinued in several markets in 2021, making long-acting formulations the primary focus of modern care plans.
Important Note: Interferon therapies are contraindicated for patients with severe liver disease or active autoimmune hepatitis, as these conditions can worsen during treatment.
Recognizing common and serious side effects
While interferons are effective for long-term MPN control, they carry potential side effects that require ongoing monitoring:
Common side effects:
Flu-like symptoms: Mild fever, fatigue, chills, muscle aches, and headache (often managed by taking doses at bedtime or using over-the-counter pain relievers as advised).
Redness, swelling, mild bruising, or irritation at the injection site.
Hair thinning, dry mouth, mild nausea, skin changes, and weight fluctuations.
Serious side effects (must be reported immediately):
Irregular heartbeats (arrhythmias) or heart muscle weakness.
Signs of liver toxicity or kidney dysfunction.
Severe mood alterations, depression, or peripheral neuropathy (nerve pain/tingling).
Significant drops in white blood cells or platelets beyond intended treatment goals.
