Bile duct cancer may not cause noticeable symptoms when it first develops. Symptoms often begin when a tumor grows large enough to block the flow of bile or affect nearby organs. The symptoms can also differ depending on whether the cancer begins inside or outside the liver.
THE BASICS: Common bile duct cancer symptoms include jaundice, itchy skin, dark urine, pale stool, abdominal pain, unexplained weight loss, nausea, and fever. Jaundice is particularly common when a tumor blocks a bile duct.
What are the signs and symptoms of bile duct cancer?
Jaundice
Jaundice means yellowing of the skin or whites of the eyes. It happens when bilirubin builds up in the bloodstream because bile cannot drain normally. Jaundice is one of the most common symptoms of extrahepatic bile duct cancers.
Itchy skin
Bile salts can build up in the skin when bile ducts are blocked. This can cause intense itching, sometimes before jaundice becomes obvious.
Dark urine
Excess bilirubin can pass into the urine and make it appear dark brown or tea-colored.
Pale or clay-colored stool
Bile normally gives stool its brown color. When bile cannot reach the intestine, stool may become unusually pale, gray, or clay-colored.
Abdominal pain
Some patients experience pain in the upper right side of the abdomen or beneath the ribs. Pain may be more common with intrahepatic bile duct cancer because the tumor grows within the liver.
Unexplained weight loss
Cancer can affect appetite, metabolism, digestion, and the body's ability to maintain weight.
Loss of appetite
Some patients notice that they are less interested in food or feel full sooner than usual.
Nausea and vomiting
Bile duct obstruction and other digestive changes may cause nausea or vomiting.
Fever
A blocked bile duct can sometimes lead to an infection called cholangitis. Fever, chills, abdominal pain, and jaundice can be signs of a serious bile duct infection that requires urgent medical attention.
Fatigue
Cancer, poor nutrition, liver dysfunction, infection, or anemia can contribute to significant fatigue.
Do symptoms differ by bile duct cancer location?
They can. Perihilar and distal bile duct cancers commonly block bile drainage, making jaundice and itching more likely. Intrahepatic bile duct cancer may grow within the liver for some time before blocking a major duct. Symptoms may include abdominal pain, weight loss, fatigue, or a mass found on imaging.
When should you see a doctor?
Talk with a healthcare provider if you develop unexplained jaundice, persistent abdominal pain, unexplained weight loss, dark urine, pale stool, or persistent itching. Jaundice does not always mean cancer, but it should be evaluated promptly.
